R03 is Amex converting an inquiry into a chargeback because the merchant's response did not adequately address the cardmember's claim. The merchant answered, but with missing, irrelevant, or incomplete documentation, so Amex resolved the case in the cardmember's favor.

What is the difference between R03 and R13?
R03 means you replied to the inquiry but the reply was inadequate. R13 means you never replied at all. Both convert the inquiry into a chargeback, but R03 at least leaves a record you engaged.
